Mplayer grabs the video from the TV card and runs it through it's own layer before putting it on the screen, the audio, on the other hand, is passed through a cable from the TV card to your sound card (in most cases) and thus there is a delay, depending on the speed of your machine between the audio, and the video showing up the screen since the video must pass through mplayer first.
