A semen analysis evaluates many aspects of the semen, including the sperm, which comprises approximately 1 to 2 percent of the semen volume. The remainder of the fluid is composed predominantly of secretions from other male glands. These secretions are also tested, as they can also affect the sperm's movement and function.

Semen Analysis when improperly or incompletely performed may miss significant problems. Unlike many other tests semen analysis relies completely on the expertise of those performing it. Ideally the laboratory should have facilities to do certain additional tests on the same semen sample.

Semen analysis generally refers to a sperm count and motility check only, this is used for diagnosis of fertility. Semen Cytology is a brand new term which describes fertility diagnosis and advanced testing of the other components in semen, similar to a PAP smeer in women.

The lab will provide instructions to abstain from sex for a certain period of time before the test, and it will give you a small jar to use for the specimen. After masturbating and ejaculating into the jar, you must take the whole specimen immediately to the lab. You may be asked to produce a specimen at the lab, where they will provide a private room.

The Andrology Lab at Men’s Health Boston performs semen analyses, and can also perform additional sophisticated tests. All tests must be scheduled by calling our office at 617-277-5000. The sample can be produced at home and brought into the lab at a scheduled time, or can be produced in privacy at our office. Semen quality changes day to day, week to week and month to month. There are also certain minor variations in laboratory techniques that will result in differences in evaluation of the semen quality from one office to another. However, the differences should not be sufficient enough to make an abnormal semen normal or visa versa. Usually, when the sperm production has once been adversely and seriously affected, one's sperm count stays in the low range, no matter where it is tested.

The quality of the semen analysis is highly dependant on the method of collection. The ideal number of days to wait after sex to give a semen analysis is three to five days. Giving a semen analysis in less than three days after an ejaculation produce a small volume low count semen sample. Giving a semen sample after greater than five days from ejaculation can cause the sperm to have decrease motility.

The quality of a semen analysis is very highly dependent on the processing technique. Method of collection, time of specimen drop off, temperature of the specimen in the laboratory, pippeting technique, and counting system all can introduce error into the results. Daily controls by the lab is very important to standardize the process of semen analysis. It is hard to compare semen analysis between two laboratories unless both labs follow the recommended processing technique.

While a normal semen analysis cannot absolutely guarantee fertility, abnormal results usually do reflect an underlying problem. Nonetheless, there are no "cutoff " values that can definitively predict who will conceive. Generally though, men with "good" semen analysis results will usually be fertile and conceive at higher rates than those with suboptimal results.

When the hair specimen arrives at our laboratories, it is inspected then assigned a laboratory control number and recorded along with all pertinent patient information in our computer database. Afterwards, the sample is finely cut, then weighed accurately to within one-thousandth of a gram of the required weight. The weighed specimen is then placed into an individually marked sterile centrifuge tube in preparation for a precise temperature-controlled microwave/acid digestion process.
