The quickest way to obtain a copy of your Veterans Discharge Papers is to visit this office. You must bring with you several forms of identification including at least one picture ID. The charge is $2.50.

After discharge from the military, veterans place their military discharge (Form DD-214) on file with the County Clerk's Office, in the county in which they reside. Veterans receive their first certified copy of the DD-214 free of charge, additional copies are $1.00 per copy. Military Discharge are NOT public records and can only be obtained by the individual (with I.D.), family member with a death certificate, or the funeral home. You may order the record in person or through the mail.

Divorce papers would be located in our Records Division, Room #101. Records Division may certify these documents for a fee.

You may search for the cross reference or microfiche number by clicking on the Service Discharges link on the Recorder's Home Page and searching the database by name. Once you have this reference number, you may contact the Recorder's Office at (937)225-6494 and request a copy. The National Personnel Records Center (NPRC) in St. Louis offers an electronic method for veterans to obtain documents from their official record that is permanently stored in St. Louis. NPRC has a web-based application system, which can be accessed at www.vetrecs.archives.gov Use the online military personnel records request system to create a customized order form to request information from your, or your relative’s, military personnel records.

Hopefully, you registered it with your local county clerk upon returning home. If so, it would be readily available to you. If not, the Collin County Veteran Services Office can send a form to the National Personnel Records Center (NPRC) requesting a copy be sent to you. You should receive requested copy in two to four months. Yes there is. Come into our office with a copy of your discharge papers and we will complete a form that will be sent to the NPRC for replacement medals and awards.

The copies in a file can be certified and there is a $10.00 fee per file for those certified copies plus the $ .25 per page.

A request can be submitted for documents pertaining to military service by obtaining a Standard Form180. If the veteran is deceased, the next of kin may request documents as well.

The entire original DD214 is required and a Drivers License with photograph, State issued Identification Card, Passport or Military Identification Card with photograph.

There is a short form to complete and identification must be presented before a military discharge can be issued. There is no fee. To file papers, bring or mail them to the Recorder's office. There is no fee and the original document will be returned to you.

Certified copies of the death certificate are issued by the county or state where the death occurred. Typically, you can expect the certified copies to arrive within two (2) weeks time, but some circumstances may make the process longer.

When an individual is separated from military service (because of retirement, discharge from Active Duty or death), his/her Field Personnel File (containing all military and health records) is forwarded for storage to the National Personnel Records (Military).
