The surgeon makes an incision through the breastbone. He removes the old heart, leaving portions of the right atrium (the top right chamber) of the native heart behind. Then the surgeon connects the aorta, pulmonary artery and right atrium of the new heart to the remaining portion of the recipient's heart. In rare instances, a patient will have a heterotopic, or "piggyback" heart transplant. Emory is one of the few transplant centers in the country performing this procedure.

The day after the operation the eye pad is removed at home. Antibiotic drops are used to help prevent the likelihood of a postoperative infection and are used four times a day initially. If there is no successor, the farm operator might be able to find a beginning farmer. Some states have programs that help match an older farmer and a beginning farmer. The farm operator and the beginning farmer should discuss a possible strategy for farm transfer. There should be a benefit for each person as a result of a transfer plan. If no successor can be found, sometimes there are off-farm children who want to be absentee landlords. It might be possible to hire neighbors to do the farming.

Conversion of laparoscopic technique to open at the time of surgery is rare, occurring about 1% of the time. In all situations, the decision to convert to an open operation is based strictly on the patient's safety. Reasons for the surgeon to elect to convert to the open procedure include bleeding problems during the operation, or inability to visualize adequately.

We understand that this is a major concern for patients and acknowledge that most operations lead to some degree of postoperative pain or discomfort. We take great care to minimise postoperative pain using a variety of techniques. At operation we infiltrate local anaesthetics into wounds and provide patients with pain killers to go home with. We recommend that these be taken regularly for the first 48 hours.
