You can request a replacement testamur (graduation certificate) if your original testamur has been lost, damaged or stolen, or if you have changed your name. To obtain a replacement testamur, you must complete and submit an application for a replacement testamur form (PDF 79.2KB). Applicants are advised to carefully read the information on the first page of the application form. Please note that it takes approximately three weeks to process the request and post out the replacement testamur.

You should notify the Registrars. Restriction will then be placed on the missing share certificate in case of any attempted fraud. A letter of indemnity will be sent to you to sign. Once completed and returned to the registrars a replacement certificate can be issued. There is a charge for this service. The Registrar keeps a record of your name, address and the number of shares you own. If your certificates are lost, stolen or destroyed, the Registrar can arrange for a duplicate certificate to be issued. Simply contact the Registrar as soon as you can, quoting your account number if possible. The Registrar will need to arrange for you to complete an indemnity covering the company for any loss arising from a dispute over who owns the shares, and there may be an insurance premium payable.

Most of the students who receive a certificate do not perform academically at levels necessary for a GED (General Education Development). A - The basic details on every CO application is not listed in the BIS system. You can obtain a complete copy of the CO by bringing the CO number to the DOB Borough office where the property is located.

The company will need to complete the application for Certificate of Authority, which may be downloaded from the Corporations Division website at: www.sos.state.ga.us/corporations. The Certificate of Authority should be submitted along with an original Certificate of Existence, which is obtained from the "home state" of the corporation and must be no more than 90 days old.

After all the exams are passed successfully you should become a member of a professional association - member of ECCAA (if you are not yet). For those students transferring in 60-80 hours who know what their degree program is, a graduation evaluation can be obtained after you have complete one semester at Western. For students with 80-plus transfer hours, a graduation evaluation can be obtained during your first semester at Western if you know what your degree program is. The graduation evaluation will provide you with a list of the courses you still need to complete to earn the BA degree.
