If you are enrolled in the Student Accident and Sickness Insurance Plan offered through the university you can obtain a claim form from Health Services or on-line at www.academichealthplans.com/trinity. Follow the directions on the form. Fill in all the blanks even if some of your responses are “not applicable”. Attach bills and statements related to the illness or injury. Copy all the forms that you send to the insurance company. You may need to refer to them later.

No, you are no longer required to file with your insurance company before filing with the Air Force. If you choose to do so, that information must be disclosed when your claim is filed with the government. You cannot receive compensation from both sources for the same loss or damage. Claims filed against the City of West Sacramento are initially reviewed by the City Manager's Office. Whatever investigation is necessary to determine if the City has any liability will be initiated upon receipt. Such investigation may entail referring the claim to a third party claims adjuster. The City has 45 days by law to respond. Any claim that cannot be settled for less than $5,000 will be sent to the City's claims adjuster for handling.

Claims for damages against the City must be filed with the City's Risk Manager or the City Clerk's Office within the applicable statute of limitations. Health Services is not a participant of any HMO, PPO, or private insurance plan. Therefore, we are unable to generate an insurance claim form to file with your private insurance. We can provide you with an itemized bill, but it is your responsibility to seek reimbursement from your insurance carrier. There is no guarantee that charges incurred at the student Health Services will be reimbursed to you by your insurance carrier.

You contact your insurance agent and file a claim as you normally would. Tell them that you have a complete inventory of the lost or damaged items and provide them with your back up copy. However, if you use our safe, off-site storage you would supply us with a signed authorization, and we will provide your insurance company with a back-up copy of your inventory. Contact your insurance agent and company right away. Any burglaries or theft should also be reported to the police immediately. Read your insurance policy so that you know what your responsibilities are to your insurance company after a loss. After a disaster, take steps to protect your property from further damage by making temporary repairs. If immediate repairs to equipment are necessary, save the damaged parts in case the claims adjuster is interested in examining them.

Get several copies of the death certificate, stating date, place and cause of death. You can obtain the death certificate from the funeral director.

After a claim is filed, the insurance company will evaluate all of the information to determine if it is their responsibility to pay. If you choose to do so, yes. We do not directly bill insurance companies, however. We can assist you in obtaining the necessary paperwork for such a claim. Be advised that our fees are so low that there is often little to no benefit in filing a claim.
