HOW DO I GET A TEAM ROSTER FORM?
FAQsYou will receive a team roster form as part of the packet you will receive when you register your team. You can also obtain a roster form by calling the State Youth Office at 215-453-9192. It's critical that all components of the roster are completed, particularly the player and parent's signatures, the insurance information and the registration (ie, sanction) number. Once completed, teams should keep the roster intact until qualifying for a National World Series.
