The waste tire manifest system was created to track the generation, movement, and disposal of waste tires throughout California. The information on the forms is entered into the waste tire manifest system tracking database, and the load data is cross-referenced. Electronic images of all CTL forms are created and are available as scanned images. These images are considered confidential.

Blank CTL forms can be obtained (free of charge) from the CIWMB using any of the means below and generally should arrive within 5?7 business days of ordering.

First, review Section 4 of this manual. You will probably find that most of your questions will be answered there. If you still have questions, you may want to review the training video/CD provided by CIWMB (see Appendix D for information on available training materials and how to obtain them). You can also contact CIWMB using any of the means described above.

The CTL maintains a library of current books and video tapes that may be checked out for campus use. Yes. An employee who is eligible for and receiving CTL may continue to use contributed CTL during the University Closure period. (CTL earnings code CTS) According to Classified Staff Human Resources Policy 203.0 and University Handbook for Appointed Personnel policy 8.01.02, University employees must work or be on an approved paid status on their last scheduled workday before and first scheduled workday after the holiday to qualify for holiday pay.

CIWMB requires registered waste tire haulers to provide the following as proof of their current registration status: Decal. There should be a current-year waste tire hauler decal affixed on the lower-right corner (passenger side) of the vehicle windshield. Certificate. Haulers are required to have a current-year waste tire hauler registration certificate in their vehicle at all times. Forms. Registered haulers must use the CTL form or be an approved participant in electronic data transfer (EDT).

After the reported data is reviewed, the forms are destroyed. The data are tabulated into statistical summaries which serve as input to private and public sectors in determination of gross domestic product, potential markets, expansion, etc.

The Form 656 and/or Forms 433 "Collection Information Statements" are necessary to conduct an offer investigation. Failure to submit these documents will cause considerable delay in the process.
