The National Archives and Records Administration (NARA) is the official repository for records of military personnel who have been discharged from the U.S. Air Force, Army, Marine Corps, Navy and Coast Guard. Paper copies of military service and pension records can be ordered by mail from the facility which holds the records. Visit National Personal Records Center.

The Veterans History Project does not hold any veterans’ records. To request replacement records you need to contact the National Personnel Records Center, Military Personnel Records (NPRC-MPR) in St. Louis. View instructions for requesting replacement records.

Submit your request on Standard Form 180, "Request Pertaining to Military Records", available at most government offices such as the Veterans Administration. The form is broken down by service...Air Force, Coast Guard, Marine Corps, Army, and Navy. A list of custodians corresponding to your category of service (active, reserve, officer, enlisted, etc.) is provided with a specific mailing address to where your records are kept.

You can obtain your own military personnel and medical records by filling out "Standard Form 180" which is available for you to print out at the National Archives and Records Administration website: http://www.nara.gov/regional/mpr.html You may also contact your County Veterans Service Officer for assistance in completing any forms for veterans services. He or she can help you get the most efficient service from the Department of Veterans Affairs.

Military veterans and the next of kin of deceased former military members may now use a new on-line military personnel records system to request documents. The National Personnel Records Center is working to make it easier of veterans with computers and Internet access to obtain copies of documents from their military files. Other individuals with a need for documents must still complete a Standard Form 180 that can be downloaded from the on-line web site.

Military records are federal records that are maintained by the National Archives. The Archives does have copies of Veterans Bonus Records for veterans who filed for a Bonus Payment after they served. These records documenting WWI, WWII, Korea, and Vietnam contain copies of the individual's discharge papers. Veteran Bonus Records are restricted but permission to view them may be obtained by the Office of Veterans Affairs.

A: To obtain a certificate in lieu of a lost or destroyed discharge, a veteran separated under honorable conditions should use Standard Form 180. Military records are found in several Record and Manuscript Groups. Click here for information on the French and Indian War, Revolutionary War, War of 1812, Mexican War, Civil War, Pennsylvania National Guard, Spanish-American War, Mexican Border Campaign, World War I, World War II, and Vietnam Conflict. Yes. There is an online name index to Revolutionary War pension records at the Pennsylvania State Archives.

Check with your family to see if they have a copy of your immunization records available. A copy of your records may also be on file with your previous place of education (high school, undergraduate university). Records may also be found at your doctor's office. Please be aware that these records may not be up to date. If you are sure that you have been immunized against the measles, mumps, and rubella, and/or the chicken pox, you have the option of getting your titers drawn.

Health records are confidential. You can request a copy of your records in writing. The letter must state that you give the College of Staten Island permission to release your immunization and/or medical records. Then give the name, address, or fax number of the agency you want to receive your records. The letter must also be signed and include your student identification number. The letter can be faxed to us or you can go to the Health Center and fill out a Medical Record Request Form.

A medical records release form is required to obtain a copy of your medical records. Records will be copied and mailed to you after the release form is received. There is a fee for this service. Please contact our office at (920) 730-8833 or toll free at (888) 383-3039 for more information.

Patients may request a copy of their medical records for their personal use or for delivery to a physician outside of Urology San Antonio by submitting the Medical Records Request form found on the Medical Records page of our website.

Talk with your dentist about getting a copy of your dental records. Dentists covered by the HIPAA privacy rule are required to provide patients with a copy of their records and state law may also apply.

The Broward County School District is a GED testing facility. All requests for verification of GED test scores and official GED transcripts are handled by the Florida Department of Education, GED Testing Office through their Web site (http://www.firn.edu/doe/workforce/ged_dipl.htm) or call them toll free at 877-352-4331.

This is accomplished by filling out the appropriate Authorization for Release of Medical Information form. This form is found in the Main Lobby at the scheduling desks or by clicking on the link. In order to process your request in a timely manner, the following information is required: The name of the hospital or doctor from where the records are being requested, a complete address and/or fax number, and a phone number. In addition, your name, address, and ASU ID# are also required.

If you do not have a copy of your discharge papers, service records, or medical records you can obtain them by filling out a Standard Form 180 - Request Pertaining to Military Records. You can download a copy from this site or obtain a copy from your local State Division of Veterans' Affairs office or the United States Department of Veterans' Affairs.
