Health records are confidential. You can request a copy of your records in writing. The letter must state that you give the College of Staten Island permission to release your immunization and/or medical records. Then give the name, address, or fax number of the agency you want to receive your records. The letter must also be signed and include your student identification number. The letter can be faxed to us or you can go to the Health Center and fill out a Medical Record Request Form.

Most immunization records are available on computer and can be printed out immediately. Microfilmed records can take up to two weeks to retrieve. There may be some instances where records are no longer available.

Out-of-state students who wish to receive their immunization records from UNO must send in a request letter along with the fee of $3.00.

Yes, Please put "Attention: Health Center, 1C-112" on the envelope and keep a copy of the records mailed to us. Measles, Mumps, and Rubella Immunization given on or after your first birthday (measles on or after January 1, 1968, and mumps and rubella on or after January 1, 1969) are good for life. However, NYS law requires two proofs of measles inoculations, so you may have to get another vaccination or submit positive titers.

It is recommended that you submit the Immunization Record form prior to Orientation attendance. The deadline is March 1st for Spring Semesters and October 1st for Fall Semesters. Students are given a grace period for the semester to which they are enrolling, however if a completed Immunization Record is not on file with Student Health Services by the identified deadline, students will be prevented from registering for their upcoming semester's classes.

No. Unfortunately Scottsdale Unified School District was not required to keep copies of immunization records prior to 1992. The new school will have you sign an authorization to release records. We will handle the transfer of records by mail. To enroll your child at any school, you will need to provide a certified copy of his/her birth certificate and proof of current immunizations at the time of enrollment.

Patients may request a copy of their medical records for their personal use or for delivery to a physician outside of Urology San Antonio by submitting the Medical Records Request form found on the Medical Records page of our website.

Talk with your dentist about getting a copy of your dental records. Dentists covered by the HIPAA privacy rule are required to provide patients with a copy of their records and state law may also apply.

The Broward County School District is a GED testing facility. The immunization program will accept official immunization records from another state or country. The information from those records will be transferred to a the Central Valley Immunization Information System (CVIIS).
