Finally you can disable the default toolbar under View > Toolbars > All-in-One Sidebar Toolbar. Please note that it's possible that some toolbar items do not work correctly when using them in the tab bar or in a toolbar below the tab bar (e.g. some items of the Google Toolbar and TMP's "Closed Tab Bin").

But nevertheless you have two possibilities to get doubled buttons. You can use Custom Buttons² or Customizable Toolbar Buttons to create a second button for one job. Use following workaround, which is actually a bug (I believe it's a Firefox bug, but I'm not really sure. :-)) configure the other toolbars to your needs.

And because I can't see any problem I have to assume that there are problems/errors in your profile. Please try to force the showing of the toolbar by disabling and enabling the option View > Toolbars > All-in-One Sidebar Toolbar. If No. 1 and 2 doesn't help I have to assume that something in your localstore.rdf is broken. This file stores customized data on the interface and maybe it's corrupted.
