This form must be completed and returned to the University prior to classes beginning. All information must be provided on the University form; information supplied by any other means (e.g., faxes, E-mail, separate pieces of paper) will not be considered acceptable and may be returned to students. If you do not complete the immunization requirements, you will not be able to attend classes until you do.
