This form can be printed off of RAIN or completed at the Registrar's Office, Sanford Hall, Room 210. If you attended another school/institution after leaving GSW, you will need to request official transcripts be forwarded to the Registrar's Office, Georgia Southwestern State University, 800 Georgia Southwestern State University Drive, Americus GA 31709-4379.
