Call the Social Security Administration FRAUD HOTLINE to notify them of the loss and get information on how to get a duplicate card.

Can a patient's Social Security number be used as a patient account number when paying medical bills? I was under the impression that Social Security numbers were not supposed to be used as identifiers. Is there a way to get a different Social Security number? I'm a victim of identity theft and the imposter is opening up new accounts using my name and SSN. [or] Someone has stolen my wallet with my SSN in it.

If your card is lost or stolen, immediately contact Bank of America at 1-800-472-1424 and notify your LAPC/APC. Depending on your agency's policies, your LAPC/APC may request a new card from Bank of America or Bank of America may automatically issue you a new card. Please check with your LAPC/APC on your agency's specific policies. I have a new account after my previous account was closed because of a lost or stolen card or security fraud.

To prevent any fraudulent activity, please call us immediately at 1-800-955-7070. If you are a small business credit card customer, please call us a 1-800-867-0904. From outside the United States, please call our collect number at 804-934-2001. If possible, please have your 16-digit credit card number, the circumstances of the loss or theft such as date and location, the last authorized charges to your account, and any access or purchase checks numbers that need to be honored.

If your EBT card is lost or stolen, you should report it IMMEDIATELY by calling your State's toll-free customer service number. A new card will be reissued to you within 2-5 days.

Call the toll-free Indiana Debit Card Customer Service Center, 1-888-393-5866, to report a lost or stolen card. A new card will be issued and any remaining balance will be transferred to the new card. If you report the missing card immediately and have not compromised your PIN, you will not be responsible for any confirmed fraudulent activity that occurs on your card.

Lost or stolen ATM or Debit Cards should be reported immediately by calling 1-800-666-4417 to minimize any possible losses. We will deactivate your ATM or Debit Card and issue you a new replacement card.

PCC is not responsible for lost or stolen cards or key tags. We ask that you and your supporters treat your cards and key tags like cash. It is your organization's responsibility to keep track of your cards and key tags and their assigned numbers.

Children under 18 years of age usually do not require photo ID for domestic flights; however, as each airline has its own requirements, please contact your airline directly to confirm its ID requirement for minors. If you do need to obtain a photo ID for you or your child, they can be obtained from a number of suppliers including www.goipass.com.

You should place a free “fraud alert” on your file with one of the three major credit monitoring agencies. (You need only to place a fraud alert with one of the agencies; it will share the information with the other two.) The fraud alert stays on your record for 90 days. You can re-file the free fraud alert after the initial period has ended. Step-by-step instructions can be found at the Fraud Alert, Data Theft and Identity Theft Resources page.

Keep a separate copy of your Gift Card number in case of loss or theft. If your Gift Card is lost or stolen contact us immediately at the Customer Service Number. Your Gift Card can be replaced, free of charge, ONLY if you can provide us with your Gift Card number. Funds debited from a lost/stolen Gift Card prior to our receipt of your notification cannot be replaced. See Agreement for more details. The Gift Card has a “Valid Thru” date on the back of the Gift Card.

If this happens, remember that it would be difficult for anyone other than the cardholder to use this information because of the calling restrictions set up by the card owner. We suggest that you make copies of the SPARC Card so that you'll have 'extras' available in case the card gets lost. It is important for card owners and cardholders to protect the Toll-free Access Number and PIN information the same way they would protect cash.

Ask for an application (Form SS-5) to apply for a duplicate card. You'll have to submit the completed application and show evidence of your identity.

Social Security Disability Insurance. The amount of your monthly disability benefit is based on your lifetime average earnings covered by Social Security. If you would like an estimate of your disability benefit, you may request a Social Security Statement that displays your earnings record and provides an estimate of your disability benefit. It will also include estimates of retirement and survivors benefits which you and your family may be eligible to receive now and in the future.

Call your local branch immediately. The after hours hotline is 1-800-500-1044. Note: A new card will not be automatically ordered for you. You will need to request a new card.

Like your ATM or credit card, if you lose your OnFile card just contact our Customer Service at 800-650-8830 and we’ll cancel the Emergency Login codes on your card and issue you new ones. You can print a temporary card with your new emergency login pass codes from the “Member Profile” section and keep it with you until your new permanent card arrives in the mail.

Your BiLo Foods and Riverside service center employees will assist you in getting a replacement card, key tags, or both.

Report problems of this nature first thing Monday morning to the UNCG FirstCard Center. Replacement cards cost $15. Please refer to the Lost, Stolen or Damaged Cards section for instructions on recovery of money.

You can call the toll-free number on the back of each gift card (1-800-269-5826) or you can contact any Dillard's store to report a gift card lost or stolen. In either case you must have the gift card number.

The Cardholder should immediately contact U.S. Bank Customer Service and their Agency Program Coordinator to report the lost card. U.S. Bank has a 24-hour a day, 7 days a week, Customer Service number for reporting lost or stolen cards. The number is 1-800-227-6736. A new account will be established and a new card issued. All charges are moved to the new account and unauthorized charges will be cleared by subsequent credits issued by U. S. Bank Fraud Investigative Services.

Report the loss to the Campus Police and Campus Card Operations, located in the Student Union, Room 113. To replace the ID, students will have to pay a $15 processing fee at the Controller's Office, located on North Campus. Once the fee is paid, students take the receipt to Campus Card Operations to receive their new ID.

You must inform us as quickly as possible if your card is lost or stolen. In such a case, we will immediately cancel your card, issue you a new one and transfer your GOLF DOLLAR$ to your new card. There is a 10 GOLF DOLLAR$ or a 10 Canadian dollars fee for a replacement card.

Return to the store where your card was issued, or contact Tempo Payments, Inc. by calling 1-800-286-2273. Remember to ask for a replacement card. It is important that you contact us immediately if you believe your card is lost or stolen.
