It means, simply, "Eat first, or f**k first?" and is believed to be old Navy slang (perhaps shorthand for "You wanna get some real food or some tail first?" on first hitting dirt for shore leave). Dora's reply of "Both!" might confuse some readers, but is completely in character, if you think about it - and has a couple of alternate meanings, as well.
