Each parent must fill out the emergency contact/registration form in its entirety, keeping the original for your records and returning a copy to EDS. The child's physician must fill out the medical form in its entirety and sign it. All forms, with the exception of the health form, must be returned prior to the child's first day in the program. The health form must be returned within one month from the child's first day in the program. These forms are required according to licensing regulations.

International students who wish to study in Canada for up to 6 months may need a Visitor's visa depending on the country for which they hold a passport. Contact the Canadian Embassy nearest you for further information. As a school, we do not decide who needs a Visa and who doesn't. The Canadian government decides who needs a Visa to visit, study or work in Canada. Citizens of many countries do not need a Visa to come to Canada for stays of six months or less.

It is not required, but would be helpful. At the secondary level our counselors are often busy and making an appointment in advance will save you time.

Treatment regiments are customized for individual patients. Generally, for out of town patients, we usually recommend daily treatments over a five-day period. For in town patients, we usually recommend a treatment every two to three days initially. Muscle memory is 72 hours and repeating treatments within that timeframe supports muscle re-education.

If the answer to all questions on the Annual Objectivity in Research Questionnaire is "no," then you only need to complete that form. If any of the answers is "yes," then you will need to fill out and submit the Investigator's Disclosure of Financial Interest Form I to the ORSPA Compliance Office at mail code 3503. If a possible conflict is identified, you will be asked to complete an additional form.

SUNY Oswego requires you to complete the FAFSA (Free Application For Federal Student Aid) and TAP (Tuition Assistance Program) applications for financial aid consideration. Priority dates are April 1 for the fall semester and Nov. 1 for the spring semester. Most students will have already filed these applications with their current schools and only need to have the processing centers add Oswego as the new school of attendance.

Yes. We will ask them for a reference or school report after receiving your application, so you should always tell them that you are applying.

Contact your professor, especially if you know you will be absent for several days. Otherwise, you can be dropped by your professor for too many absences. Call the professor at his/her office phone listed on your syllabus. If the professor is not in the office, leave a message by phone or e-mail clearly stating the reason for your absence and your name and social security number. You must be prepared to verify that you have contacted your professor and provide documentation of emergencies.

The bus program can accommodate up to 70 children in one school day. If you can identify at least fifty to 3rd, 4th, and/or 5th graders from your school or in combination with other schools in your district, then the bus can come to you. There may be exceptions to this, so please contact us to discuss your situation.

Handwriting is a vital tool to communication and learning in the classroom yet many schools no longer teach handwriting as part of their curriculum. Emphasis is placed on state mandated subjects such as reading and math, and without proper instruction in handwriting, children’s letter formation is often inefficient.

Many countries have a visa waiver arrangement with Japan, allowing you to visit for a certain period (usually, 3 or 6 months) without any visa at all.
