We require payment in full two weeks prior to the date of surgery. There is a company we work with called Health Finance Options who handles financing. Usually we can fill out an application in the office and have a reply within 24 hours. We also accept VISA and MasterCard.

Birth and Family Place is negotiating with all major insurance carriers in the Salt Lake area. Ask your insurance carrier if they have a contract with Birth and Family Place. For the 89-90% of women who have normal births, the birthcenter is a safe, low-cost alternative to hospital stays. Until we are covered by your insurance, we accept cash or credit card payment in advance to cover the $2000 facility fee.

Unfortunately, we cannot accept payment plans. However, we do accept credit cards, which offer you the ease and flexibility of monthly payments.

We do not file or accept any paper work for insurance on any "cosmetic procedures," laser service, or peels. We accept Visa, Mastercard, Checks, or Cash. Payments can be made through Google's Checkout, PayPal, or check. All funds must clear before coaching begins.

Unfortunately, not all pharmacies will accept the checks. You may want to ask a different pharmacy if the savings checks for LAMICTAL will be accepted.

We have a variety of different payment plans available, including one that will be custom made just for you. We also accept assignment of all insurance plans.

Please call to confirm which insurances we accept. We are involved with the credentialing process with a number of common carriers and will have the most updated information over the phone. States do share information regarding large purchases shipped or delivered to other states.
