The standard GIDEON diagnosis matrix is Bayesian; ie, based on the product of disease incidence X symptom/sign probability. In contrast, the Bioterror module is non-Bayesian - ie, relative disease incidence (prior probability) is not factored into generation of the differential diagnosis list. Although we may intuitively assume that Anthrax and Smallpox carry higher 'probability' for use in Bioterror, vs. Marburg disease or Argentine hemorrhagic fever, precise numbers are lacking.

The diseases listed are compatible with the signs and symptoms selected, but are not endemic to chosen country. This tool was developed in consultation with the World Health Organization, and is designed to identify the initial cases of known diseases in new settings; ie, SARS in Canada (2003), Monkeypox in the United States (2003) West Nile fever in the United States (1999), Japanese spotted fever in Korea (2004), etc. For an example of use, see First Case Scenario.

If a man is found to have an elevated PSA or an abnormal DRE, a prostate biopsy is recommended. This is a painless 5-10 minute procedure most often performed in the office under local anesthesia. Most patients are able to return to full activity on the day of the biopsy. When the US defence department developed the GPS satellite system, it was meant for military applications - to help pinpoint the exact position of a soldier in the field, for example. That system is extremely accurate, within something like 50 feet. The DoD didn't want to give that capability to the enemy, so they scrambled that signal. The signal that's available to the public has been deliberately tweaked to provide a position within about 200 feet.

Spammers can trick most systems you put in place, but delivery of their commercial message works against them when a Bayesian filter learns from subject and message body content, applying acquired knowledge to all future email's

Bayesian Filters recognize spam by looking at the words they contain and scoring words on a point based system. A Bayesian filter starts with two collections of mail; one of spam and one of legitimate mail. For every word in these emails it calculates a spam probability based on the proportion of spam occurrences. In my own email "Guaranteed" has a spam probability of 98% and assigns a high point score to this word because it occurs mostly in spam.

A scenario is the Trainz version of Microsoft Train Simulator's "activity." Both are a set of tasks for you, the engineer, to complete. In use, the instructions are displayable while driving. It's possible to create your own activity or scenario; both are somewhat involved. MSTS has a built-in activity function, whereas Trainz requires some knowledge of scripting. Control unwarranted fears that tax the public health system and help terrorists attain their goal of spreading panic. Improve our national disease surveillance system, enabling rapid response, containment, and treatment. Develop stockpiles of needed pharmaceuticals to be used in the event of a bioterror incident, so that if a huge number of people are affected, supplies will be adequate to provide treatment.

A number of different bacteria, viruses, and toxins could be used as weapons of bioterrorism. Salmonella was used as a food poisoning by a religious cult in Oregon in the early 1980s to sicken hundreds of people. Biological weapons programs in the Soviet Union explored the use of smallpox, cholera, and plague. Among the 28 agents developed by the U.S. biological weapons program of the 1950s and 1960s were the agents that cause tularemia, Q-fever, and Venezuelan equine encephalitis.

The best defense against bioterrorism is a strong public health system. In response to the threat of bioterrorism, Congress authorized the Centers for Disease Control and Prevention (CDC) to coordinate efforts to upgrade national public health capability to counter bioterrorism. Following this mandate, the CDC established the Bioterrorism Preparedness and Response Program in 1999.

A doctor is the most qualified to diagnose hemorrhoids. He will examine the anus and rectum and note the formation of enlarged blood vessels. He can also do a digital rectal exam. There are also tools to diagnose hemorrhoids such as the anoscope and protoscope. The lighted tube anoscope is particularly useful in inspecting suspected internal hemorrhoids. The protoscope, on the other hand, is used to examine the entire rectum.
