You must submit a written request for the report through the US mail. The written request must contain: Date of incident, time of incident, location of incident, parties involved, incident report number (if known).

NIOSH arranges for experts to review your chest X-ray, and stores the film for future comparisons. If you need a copy or want more information about your chest X-ray, contact NIOSH (see enclosed resource list).

Call or visit the local police agency or precinct where the accident occurred. Ask the agency if your report is available and what the fee is.

To obtain a copy of a police report you must first fill out a Freedom of Information Act request at the office of the City Clerk. The City Clerk's Office is located on the first floor in City Hall. Police reports cost $0.25 for each side of the page (most reports have 2 or more pages). Please note that it may take up to seven working days for a report copy to be obtained.

In accordance with the Freedom of Information and Protection of Privacy legislation and the Hospital’s Fees Policy, a patient may request a copy of their MRI report by completing our Request for Reports form. You can obtain copies of traffic accident reports of incident reports by visiting our Department between the hours of 8AM and 5PM, Monday through Friday. We are located at 801 North Jordan Ave, Bloomington, IN 47405. There is a nominal charge for copies of traffic accident reports and incident reports.

The volumes are out of print, but are available to read at US Government depository libraries throughout the US. Go to http://www.gpoaccess.gov/libraries.html to find a depository library near you. The National Archives and Records Administration (NARA) sells microfilm copies of the Warren Commission Report and its 26 supporting volumes. A simple request, in writing, to the Chief Fire Marshal is all that is needed. Depending on the severity of the fire, as well as the overall workload of the Division, it can take from several days to several months for the report to be completed. As soon as it is finalized, your request is honored. There is no fee for this report.

While Incident Reports are substantially matters of public record, we do take precautions to protect the privacy of individual homeowners/occupants. Some entities are legally entitled to the Incident Report without question: other Town agencies, Law Enforcement agencies, insurance companies and their private investigators, as well as contractors, owners, workers or others who may be involved in or affected by the fire, and those who obtain subpoenas or court orders.
