Open the map image file using the file -> open menu item. Ensure that the image resides on writeable media as the georeference data is saved in a file in the same location. Press the georeference map button to start the georeference dialog. This could be one of a number of problems, but the most frequent is a browser problem... Some browsers don't recognize the HTML code <INPUT TYPE=IMAGE...>. The iMAP and Parcel Viewer main map displays contain a separate, unique GIF image that is regenerated whenever the display redraws. When using a 3D render software, you should always use the eight views approach. This will result in best possible images. A 3D image, made of colour image + depth map, doesn't contain information about object edges and will always result in a "relief" of a 2D image.
