Exam Papers are now available online. Copies of exam papers from the last five years are available from the Short Loan Collection (SLC) on Floor 2.

You should make 3 sets of copies of your passport, airline tickets, credit cards (that you will be using) and travelers checks. Keep one copy in a different piece of luggage, one copy should be at home or at the office and give one set to the faculty for the course.

In Oregon, full divorce records must be obtained from the County Court that granted the divorce. In Clackamas County, copies are available in the Courthouse File Room, 807 Main St., Oregon City, OR 97045, Room 12. (503) 650-3036.* Divorce records older than 4 years should be requested 24 hours in advance of date needed to provide for retrieval from off-site storage. The Oregon Health Division, Center for Health Statistics (and Vital Records) can provide you with a summary of a divorce record.

The Department of Veterans Affairs does not maintain a record of veteran's military service, except as is necessary for providing benefits. For information about obtaining your military record, you can go to the website vetrecs.archives.gov. That page gives instructions in submitting a SF 180 Request Pertaining to Military Records or you can come to or call the Johnston County Veterans Service Office for help.

The adoptive parents or child may request a Certificate of Adoption which states the date and place of adoption, the child's birth date, the names of the adoptive parents and the name the child has taken. A release of any further information requires an order from the court. Forms are available in the clerks office to petition the court for information.

All typed assignments should be given to me in two forms-paper copy in class when it is due, and an electronic copy. Electronic copies should be complete and in a single file -your name should be on the paper, the title should be included, etc. I should be able to open up the paper on my computer and see the same thing you turned in on paper. The quickest way to obtain a copy of your Veterans Discharge Papers is to visit this office. You must bring with you several forms of identification including at least one picture ID. The charge is $2.50.

If you do not have a copy of your discharge papers, service records, or medical records you can obtain them by filling out a Standard Form 180 - Request Pertaining to Military Records. You can download a copy from this site or obtain a copy from your local State Division of Veterans' Affairs office or the United States Department of Veterans' Affairs.

One copy of each should be brought to the Office of the Registrar on the specified due date. On the same day, each member of your Moderation Board should also be given one copy of each paper.
