All of our medications are prepackaged and labeled with our physicians name. Only prescriptions written by our physician or nurse practitioner can be filled in our pharmacy. However, if you are currently on an oral contraceptive that we sell in our in-house pharmacy, we ask that you provide us with your pharmacy name and phone number. Your prescription can be transferred to our pharmacy and be filled here.

We strongly recommend that you carry your identification (ID) card with you at all times and that your family members carry their ID cards with them as well. You must have your ID card available when calling for an appointment and each time you receive services. To ensure prompt payment of claims, make sure that the information on your ID card is correct and that all providers have the correct date of birth and the spelling of your name.

Simply present your UCA card at a participating provider to receive UCA discounts, just as you would at home. We suggest that you use the Provider Locator in advance to locate providers in the area you are planning to visit.

You must complete a claim form for every accident or sickness for which you are filing for benefits. Only one claim form needs to be completed for each medical condition. For example, you visit your physician, he prescribes a medication, and later you have a refill on the prescription. Initially, you only need to submit one claim form.

If you have a prescription written by an outside physician, you can bring it in or have your physician fax the prescription to us at 315-443-7981, or call us at 315-443-5691. You should make sure we have received the fax, and are able to fill it. It's helpful if your physician puts your student ID number and birth date on the prescription.

The federal government classifies marijuana as a Schedule I drug, which means that licensed medical practitioners cannot prescribe it. Your physician must provide written certification of a “debilitating medical condition” and can only recommend the use of medical marijuana. The Montana Medical Marijuana Act states that an “attending physician" means a physician licensed under MCA Title 37, Chapter 3. M.D.s and D.O.s are the physicians licensed under this chapter.

We will be glad to complete health forms including those related to Family Medical Leave (FMLA) and disability. However, because of the time involved in completing these documents, there is a $25 service charge. This fee is due at the time you pick up the forms. You may drop off the forms with the receptionist at your urologist's office. Please allow us 24-48 hours to complete these documents.

Technically no. Both the therapists at Sandy Physical Therapy have received advanced training and certification that allows them to see patients without a referral from a physician. However, most insurance companies will not pay for physical therapy unless ordered by a doctor. Contact your insurance company to see if they require a doctor's referral. <back to top>

You'll be seeing your obstetrician, midwife, or other prenatal care specialist very regularly over the next nine months. But how do you know when to call the doctor between prenatal checkups? What's normal and what's not? If you experience any of the following symptoms, call your health care provider immediately: Significant slowing in the baby's movements after 28 weeks (less than 10 movements in a 2-hour period).

Most traditional doctors don’t do this testing because they don’t have the experience or training, and are too busy or skeptical to invest the time to learn. Dr. Blum received her FM training with Dr. Bland, and continues to work as a FM practitioner, and has much experience using and interpreting these special tests.

If we have seen your pet recently for a certain condition you may call to inquire if you may get a refill for its medication. If the doctor OK'AYS it, we will be glad to refill it and you can pick it up at any time. If we have never seen your pet, or if we have not seen your pet recently (typically if it has been over a month, depending on the illness) we will NOT refill or prescribe any medication without seeing your pet.

To better serve you in your time of need, all requests for prescriptions will be called into a pharmacy within twenty-four hours of receipt of the call. We will also try and find a pharmacy that is convenient for you.
