There are a few tips regarding this subject on the forums. PS/2 apparently is not recognized by OS X if there is a USB mouse attached. The easiest solution is to use USB keyboard and mouse, or some mice include an adapter to allow them to operate off the PS/2 mouse port. Basically, you need to be all one way or the other for the OS to function correctly. Alternatively, people have found that sometimes going into BIOS and disabling the USB (legacy) keyboard and mouse settings work.

Now when you select a specific note, you can affect the velocity and pitch by triggering your controller. So this does what you want basically, but it alters both pitch and velocity together. Most hosts should work without changing any settings, but some hosts use keyboard hooks to get the keyboard input and this steals it from reViSiT. So Manual mode steals it back!
