You had both JRE 1.4.2_10 and JRE 1.5 on your computer and you ran the CD and installed CSI. Then you uninstalled JRE 1.5 (possibly to use the accessibility option) and ran the CD again and no further software was installed. When opening the CSI management utility, a Windows error said it "cannot find the CSI shortcut".
