For those events that occurred in Kansas certificates and records are available only from the Office of Vital Statistics, an agency of the Kansas Department of Health an Environment. You simply fill out the proper form (which the Office can provide you) and return it in person or by mail to the Office. Identification may be required, and expect to be charged a nominal fee for each copy requested.

No, those for England and Wales are held by the General Register Office (part of the Office of National Statistics). You can order certificates online or download order forms on the ONS website - you will also find information on fees for their postal service.

You may only receive a relative's birth record if you are doing genealogical research and the person is deceased (a special application form and proof of his/her death is required) or the person is 75 years of age or older. Birth records for genealogical purposes may be requested from either IDPH, Division of Vital Records or from the county clerk. Records filed prior to 1916 must be obtained from the county clerk's office in the county where the birth occurred.

Certified Birth, Marriage or Death certificates for events that occurred in the Town of Dover, Morris County, are available for purchase at the Dover Health Department. Restrictions apply. For information click "Birth, Marriage, Death Records."

Births, deaths and marriages are recorded by the province through the Office of the Registrar General. Before getting a birth certificate, you must register the birth with the City. See Registry Services birth registration. Marriage applications must be submitted in person at a civic centre, but can be downloaded ahead of time.

Idaho began recording deaths in 1907 at the county level; however, not all deaths were recorded. The deaths that were recorded can be found at the Historical Library in the individual county records on microfilm. (They can also be found at the County Recorder's office in each county.) Beginning July 1, 1911 and continuing to the present, deaths have been recorded at the state level.

A marriage certificate may be obtained from the town where your intention/license was filled out. A copy of a death record may be obtained from the town a person died in or where that person resided at the time of his death. The Indexes are available on CD-Rom and microfiche at Queensland State Archives, the State Library of Queensland, at family history societies and at some local libraries. The indexes are not yet available online, however the Registrar-General is working towards putting them online as soon as possible.
