In California our driver can provide you with a duplicate title form to sign. In all other states, you must obtain the title prior to donating it.

An application for duplicate title may be obtained at any County Treasurer's office, or download Form MV7. You must mail the completed application and a fee of $10 to: Title and Registration Bureau, 1003 Buckskin Drive, Deer Lodge, MT 59722.
