Even if you believe that you never had a birth certificate, you should follow the instructions to request a certified copy from the Tennessee Vital Records Office if you were born in Tennessee. If no record of your birth is located, the Vital Records Office will send you a letter certifying that no record is on file. Many agencies that ask you for a birth certificate will accept this certified letter from the State of Tennessee in place of a birth certificate.

On September 16, 2002 records of births and deaths which occurred within the corporate city limits of Hurst were transfered to the Tarrant County Clerks Office for Vital Statistics, Tarrant County Courthouse, Room 180, 100 West Weatherford St., Fort Worth, Texas, 76102. If you recently delivered a baby at ACMH, shortly after the birth you completed the information that is submitted to the PA Department Health, Division of Vital Records. This submission is required to be completed within 10 days from the date of birth. Typically it takes 4-6 weeks for the State to process this information and in that timeframe the original birth certificate should arrive at the address you submitted, free of charge.
