Although we welcome walk-in patients as well as their family members and friends, we suggest that you schedule a comprehensive low vision evaluation with a low vision professional. A low vision examination is vastly different from the typical eye examination.

There are a couple of things you can do to help facilitate getting your tattoo. Most importantly, you want to be as healthy as possible when you come in. Here are some recommendations to help make things go smoothly: Eat before you come in, or else you might feel dizzy or light-headed. Do not drink alcohol or do drugs at least two days prior to getting tattooed. Inkline does not tolerate any alcohol, drug, or intoxicants in the studio.

Some appointments are released for on day booking, either online or by telephoning the University Health Service, on 22 22 100, as early as possible i.e. between 8.45am and 9.30 am. No. The physician at the Health Department will do a clinical breast exam to determine if you need a screening or a diagnostic mammogram. Then an appointment is made for you either at The Retreat in Goshen or Elkhart General Hospital.

The screening examination assesses your dental health. We review the information gathered during the exam to determine whether our student clinics can manage your case. Some patients learn at the screening appointment that HSC-BCD will be unable to meet their needs.

Most medical specialists will accept only referred patients. This is partly to try to ensure that the specialist you are seeing is appropriate for your condition, and also because a referral allows you to receive the maximum Medicare rebate.

Many Vision Rehab patients are referred from their current eyecare provider. Attached to our office brochure is a short referral form that can be quickly filled out by the doctor's staff and mailed to our office. After we receive the referral form, we contact the patient to schedule an appointment for a low vision evaluation. We ask that a low vision history form including the patient's visual goals be completed prior to the evaluation.
