A written request with your signature is required to obtain a transcript. Forms are available online or at the Admissions Office windows in the College Center. You may submit a request in person, by mail or by sending a fax to (916) 691-7467. Include your name, student ID number, date of birth, social security number, where you wish the transcript sent, and your signature. You may request two official transcripts at no charge, and there is a $2 fee for each one thereafter.

Detailed instructions and the transcript request form are available on the Registrar’s web site. There is a $3 fee per transcript and a $15 same-day processing fee if necessary.

Most universities will provide you with a student grade report or a form to request an official transcript. If you do not receive instructions for requesting a transcript, contact the university. Generally, there is a small fee for an official transcript.

That depends on your University, some require it, and others do not. Check with your University's records office.

You may request a transcript by fax, mail or come into the Registrar’s Office or online through your myLeo if you attend TAMU-C after the fall of 1993. Your request should include your name, campus wide identification number (CWID) or social security number (SSN), last year of attendance, address the transcript should be sent and your signature.

When you submit the transcript request, please indicate any special instructions on the form and we will send the necessary documents along with the transcript to the appropriate institution. Wait lists are not evaluated on a first-come, first-served basis.

Because of the volume of communications received at STTU, we can only respond to OFFICIAL training requests from personnel authorize to contract training services or specifically tasked with researching training for their team or agency. Please make initial requests in writing on official letterhead or by email to STTUHQ@aol.com

For a high school transcript, it must have the date of graduation. College transcripts must arrive in a sealed envelope from the sending institution. In order for the student to receive credit for each class, TAMU-C must receive an official transcript from each school attended. Although classes may be listed on one transcript, the school may have listed it as the course it transferred in instead of what class TAMU-C would transfer it in.

Call the MARS number (301.403.0500) or make your request via WEB (www.testudo.umd.edu). Transcripts will be mailed the day after your request has been received. There is no charge for transcripts. No because these core courses are required to meet the standards of the National Policy Board for Educational Administration and the National Council for Accreditation in Teacher Education (NCATE).

Requests for an official transcript must be in writing to the Registrar’s office. Forms can be found in the Owensboro office or you can write a request. An online transcript request form is available through the WKU Registrar's website. Payment, $4.00 per transcript, must accompany the requests. Be sure to include your name, social security number or Western ID, and the location where you want your transcripts sent.

Complete your transcript request form online. Once you have completed the form, you will be given a confirmation e-mail letting you know that the transcript has been mailed out.
