Vein ligation and/or stripping is a minor surgery that is commonly peformed on large varicose veins. The procedure can also be done to prevent venous skin ulcers from reoccuring after treatment. This procedure is performed to avoid complications of vein damage. Vein stripping is the only choice for eliminating the saphenous veins (the largest surface veins in the legs) that serve as the major channels for blood among the superficial veins in the leg.

Occasionally, the procedure can damage superficial nerves along the stripped vein, causing a patchy area of numbness in the leg. The nerves usually recover, however, and sensation returns. In some instances, it can result in complications such as infection, which will require antibiotic treatment, and bleeding, which can be treated by firm pressure.

No. Ambulatory Phlebectomy is different from the classic vein stripping procedure. A phlebectomy is localized to the surface area where the varicose vein exists. The procedure is performed under local anesthesia, the incision is very small which leaves minimal or no scarring. Patients walk out of the office, and most are able to resume work and their normal activities within just a few days. A phlebectomy is often combined with ELAS for best results.

Tying (ligating) the vein temporarily solves the problem, but studies have shown that the rate of recurrence is high. Temporary brownish discoloration is common. It is caused by a pigment called hemosiderin that is released in the skin from veins that are in the process of disintegrating. Not at all. The chance of recurrence is quite high after stripping. I see a few hundred patients every year who have undergone 1-4 strippings in the past, and they have recurrence of varicose veins in the same leg. Obviously, these are not the old veins which were removed. The reason for the recurrence is that the leaks (incompetent valves) have not been controlled during surgery.

Sometimes it's easy to determine what causes the veins to develop. The known factors include heredity, pregnancy, female hormones (especially progesterone), obesity, and standing all day at work (hairdresser, sales, etc.) Often we don't have a clue. Varicose veins develop more frequently on the left for reasons unknown.

Theoretically anyone can get a DVT after any surgical procedure. However, the risk of DVT after EVLT is very low (less than 1 in 400).

After the procedure, a compression stocking should be applied that cannot be removed except on your follow-up visits. Cast protectors can be purchased to cover the stocking and allow you to shower. No limitations are placed on normal activities with your stocking in place. On occasion, temporary skin pigmentation changes may occur following sclerotherapy. Limited sun exposure or use of 30 SPF sunscreen is advised until this staining resolves.

Shut your eyes while you are standing, and feel your legs. If you cannot feel the veins, you have a spider vein problem. If you can feel the veins, and they push in easily but bounce right back out, you have varicose veins.
