You are welcome to send your information to us in whichever way you feel most comfortable, but we ask you to send it just once. If we receive your information many times (fax, email and/or post) it increases the possibility of confusion and possible double files, formulas and billing for formulas prepared and/or sent. It is best to simply contact the clinic to confirm your correspondence has been received.

The basic idea is that your fax message is composed as an email message and you send it to a special address that includes the phone number to which you are faxing. The form of the address is: First_Last%phone@faxgate.acs.ohio-state.edu where First_Last is made from the first and last names of the recipients (or any other identifier). E.g., John Doe becomes John_Doe.

Sending a fax by email is easy. Just enter the destination number address as follows: destinationfax#@fax.send2fax.com, where “destinationfax#” is the number you wish to fax TO. For example, if you are faxing to someone at 888-555-8888, you would address the email to 8885558888@fax.send2fax.com. Then, simply type a message in the body of the email (optional), attach any documents for faxing, and hit SEND. You can view an actual screen shot example for further instructions.

You should share your concerns with your parent and remain open to considering many possible solutions to this problem, including at-home solutions. Perhaps some help in the house, meal delivery services or an emergency response system could enhance your parent’s ability to continue living at home. You undoubtedly will meet resistance if you insist that s/he immediately move in with you or to an assisted living facility.

Unless you already know an editor, or unless an editor has indicated on the publication's web site an openness to receiving queries by email or fax, it is best to send a query letter by mail. That also gives you an opportunity to enclose "clips" - one or more samples of your previously published articles.

Throughout all the years that our agency has operated, not one single case of misuse of personal information was reported to us by any client. However, for additional safety, you can use alternative ways of sending the information: for example, you can leave the last 4 digits of your credit card number uncompleted on the charge authorisation form, and send them to us through an e-mail, or even by giving us a call.

Have the person who has the rights to the music or materials sign the release form. In the case of royalty-free materials, this is the person who purchased them. In the case of public domain materials, this could be anyone on the team. Please also include documentation that shows your rights to the music or materials, such as a license, a purchase receipt, or a statement by the author. Note: You may only use materials if the license granted to you allows for all forms of distribution.

Faxes sent to your SRFAX assigned number or SRFAX 800 fax number are received by our servers. As soon as transmission has completed we then send you an Email with the fax as a graphic attachment.

Middleton Library does not scan, fax, or copy and send materials to out-of-state or non-LSU persons. Most materials may be borrowed by placing an interlibrary loan request at your home institution. Send a separate fax transmission using the special cover sheet for each “type” of document. For example, you can send all college transcripts together in one transmission and then send all SF-50s together in a separate transmission.

To send a fax, visit the Copy Center, Christensen Center, lower level. There is a fax machine available for outgoing faxes: local, long distance, and international. Cash basis only. To receive a fax, have it sent to 612–330–1649. Your full name and campus box number must be clearly stated on the cover sheet. Faxes arrive at the Incoming Mailroom, main level, Christensen Center. The fax will be put in your campus box.
