A co-payment is a type of cost sharing whereby the insured person typically pays a specified flat amount per unit of service or unit of time (e.g., $10 per visit, $25 per inpatient hospital stay, or $75 per emergency room visit) with the insurance paying the balance.

We require the copayment at the time of your visit. To make your check-out process easier, we ask for the copay at the time of check-in.

a $15 copay will be responsible to pay $15 at each visit. It is important to be familiar with your individual plan. Some plans require a copay for office visits only while others require a copay for office visits as well as X-rays, physical therapy, etc. pay your copay at your visit. (In the case of copays on X-rays, physical therapy, etc., most insurance cards do not document this copay; you may receive a bill for this amount.)

The copay must be paid at each visit before the service is rendered. If you are unsure of the amount which is expected, please call the office before your visit to verify the amount.

Non-Preferred Brand drugs will have coinsurance instead of copays. With coinsurance, you pay a percentage of covered expenses. For 30-day supplies, you will pay 30% of the cost, but not less than $35 or more than $65. For 90-day mail order supplies, you will pay 30% of the cost, but not less than $88 or more than $130. For example, if you get a 30-day supply of a Non-Preferred drug that has an actual cost of $150, you will pay $45 ($150 X 30% = $45).

A Doctor Office Copay is the price a patient pays for his or her doctor visit. In many cases, any costs from that visit are covered at 100% after the doctor office copay. Often, lab tests, Blood work ups, MRI's, etc. are not included in the doctor visit, but are covered after the deductible is met.

Some policies provide for office visits with copays (for example, $35). This means that you must pay the copay for the office visit. The copay applies instead of the deductible or the coinsurance. Once you make the copay payment ($35 in this example), the office visit is covered, whether or not you have met the deductible. Some policies require "facility payments" (also called "service deductibles") for hospital stays and possibly for outpatient services.

The number of Doctor Visits varies between health insurance companies. Some companies offer as few as 2 visits and other insurance companies may have an unlimited number of covered doctor visits. The cost for these unlimited doctor visit plans can be very expensive, however.

Generics are typically less expensive than their brand-name equivalents. The average cost for a 30-day supply of a brand drug is about $70, while the average cost of a generic medication is about $20. Many brand name drugs cost more than $100 for a 30-day supply. For this reason we are able to make generic drugs available at a lower copay amount.

We do not have access to that information and you need to contact your insurance company for clarification. Your insurance plan dictates what your copay will be and those amounts may be revised on a daily basis. Only by prescription, which we will be happy to request from your physician. New regulations for over the counter sales are difficult and inefficient to manage.

Usually a Deductible does not have to be met or paid. There may be, however, some companies where this is not true, although it would be unusual.
