The quickest way to obtain an abstract of your driving record is to go to your nearest Secretary of State facility, complete a request form, pay the $12 fee (no credit or debit cards accepted) and receive the record on the spot. Include the full name, date of birth and Illinois driver's license number if possible.

Vital records such as birth, death and marriage certificates may be requested in person or by mail. The fee is $15.00 for the first copy, and $5.00 for each additional copy of the same record (and a $5.00 enhancement fee when ordering online). Remit a check for $10.00, payable to the University of Cincinnati, and send to the address at the bottom of this page as much information as you have on the individual(s) you are researching. Response time is typically 2-4 weeks. The $10.00 fee is the minimum amount we assess and will cover 1-40 pages of photocopies. If an individual requests a copy of his health record from a health care entity, the health care entity may impose a reasonable cost-based fee, which shall include only the cost of supplies for and labor of copying the requested information, postage when the individual requests that such information be mailed, and preparation of an explanation or summary of such information as agreed to by the individual.
