The National Archives and Records Administration (NARA) is the official repository for records of military personnel who have been discharged from the U.S. Air Force, Army, Marine Corps, Navy and Coast Guard. Check with your family to see if they have a copy of your immunization records available. The letter must state that you give the College of Staten Island permission to release your immunization and/or medical records.
