Upper abdominal pain is the most common symptom of both acute and chronic pancreatitis. The pain is centralized and is continuous. Most patients typically say that it radiates to the back. Many say it is worse when they lie down but is relieved when sitting up. Within an hour of beginning, the pain is usually at its worst and often lasts for hours (or in some cases, for days). Occasionally there is nausea and vomiting to go along with this pain.

Pancreatitis is inflammation of the pancreas , an organ that makes the hormones insulin and glucagon. These two hormones help control the body's metabolism. The pancreas also makes other hormones and enzymes that help you digest food. Inflammation of the pancreas happens when digestive enzymes leak out of the tube (duct) that connects the pancreas to the intestines. The inflammation may develop suddenly (acute pancreatitis) or over many years (chronic pancreatitis).

Currently there is no complete cure available for pancreatitis. Unfortunately, in the majority of cases, the pancreas is not able to repair itself and return to normal functional capacity after damage has occurred. The level to which a patient can recover depends on a number of factors such as when the diagnosis is made, the extent of damage done when diagnosis is made, the success of surgery if it is performed and how well the patient commits to new dietary and lifestyle regimes.

Gallstones and alcohol abuse are the most common causes of pancreatitis. Less common causes of pancreatitis are certain infections, high triglycerides, certain medicines, and injury. Long-term pancreatitis may develop after one attack or after several attacks over many years. Long-term abuse of alcohol causes 70% to 90% of the cases of ongoing pancreatitis in Western countries.1 In some cases, the cause is not known.

A doctor diagnoses pancreatitis by asking questions about your symptoms and past health and by doing a physical exam and tests. You may have blood tests to see whether certain enzymes are higher than normal—signs of possible pancreatitis. Other lab tests may include a complete blood count (CBC), liver tests, and a stool analysis. Abdominal ultrasound can be used to spot gallstones that may cause pancreatitis. Other imaging tests that might be used include a CT scan and an MRI.

Tests are carried out for two reasons. One is to diagnose the presence of pancreatitis and secondly to find out the root cause of the problem. These help the doctor to evaluate the extent of the condition and chart out a treatment procedure for that particular patient. Most of the likely tests are described in the book. As a first step in the diagnosis your doctor will generally review your medical history.

In most cases, you will simply start with your general practitioner. He or she may refer you to a specialist if pancreatitis is suspected. Your doctor should be able to recommend exactly which doctors and specialists you will need to see to help care for your medical concerns. Your first visit will probably be to a gastroenterologist. A gastroenterologist is a physician who specializes in diseases and conditions of the digestive system. He will be able to diagnose and recommend treatment.

Normally, the structure of the pancreas and the pancreatic duct prevent digestive enzymes from damaging the pancreas. However, certain conditions can develop that cause damage and pancreatitis. Most attacks of pancreatitis are caused by gallstones that block the flow of pancreatic enzymes or by excessive amounts of alcohol.2 Sudden (acute) pancreatitis may happen after a drinking binge or after many episodes of heavy drinking.
