You will need to bring a valid photo ID. In addition only your self, spouse, brother/sister, child/grandchild, parent/step-parent, grandparent, or an authorized agent, attorney or legal representative of any of the above persons listed will be allowed to obtain a certified copy of any vital record

Birth or death certificates or copies of the same are available from the Clerk and Recorder's Office at the Courthouse, 155 West Granite, Butte, Montana, Room 201. Certified copies of birth certificates are $5.00; death certificates are $3.00.

We can mail you a form, with instructions, which need to be completed and mailed to the Office of Vital Statistics in Topeka. Their telephone number is (785) 296-1500 if you wish to contact them directly.

A certified copy of a birth certificate can only be issued by the Health Department of the municipality where the birth occurred. The Health Registrar in the Municipal Clerks office on the second floor of the Municipal Building at 5 Brophy Lane can only issue a certified copy of your birth certificate if you were born in the Borough of West Paterson. Since there are no hospitals in the Borough, there are few birth certificates on file in the Clerks office.

Certified copies of the death certificate may be obtained by contacting the Registrar-Recorder/County Clerk office by phone at (310) 462-2137 or visit their office at 12400 East Imperial Highway, Norwalk, CA 90650.

These records are not maintained by the County Clerk's Office, but rather by the municipality in which the event took place. Birth and death certificates are handled by the towns, cities and incorporated villages. For more information, click here.
