Network dentists have contracted with The Guardian to submit claim forms and accept benefits directly from The Guardian. Some non-network dentists may submit claims directly to The Guardian. More often, however, non-network dentists will require that you pay for services at the time they are rendered. Afterwards, complete a simple claim form and forward it to us along with a copy of your payment receipt.

According to the American Dental Association, adults should schedule regular cleanings a minimum of every six months. Dr. Wooten believes this schedule should be modified to meet the specific needs of the patient. Consult Dr. Wooten to find out the best dental schedule for you.

No claim forms are needed for either network or non-network care. However, evidence of payment is required for non-network benefits. In order to be sure you include all information necessary to process your claim, you may want to sign on to the VSP website and access VSP's online Out-of-Network Reimbursement Form. Please refer to the enclosed "How VisionGuard Works" brochure for details.

Delta Dental PPO and Delta Dental Premier network dentists automatically submit claim forms (hard copy or electronic) on your behalf at no additional charge. Since network dentists are reimbursed directly, when you visit a network dentist, you do not have to pay your bill upfront (only any copayments or deductibles) and wait for reimbursement. Out-of-network dentists are not contracted to submit claim forms to Delta Dental of Illinois.

You may also call ASIFlex at (800) 659-3035 and request additional claim form Payments are released for all claims processed before 2 p.m. Central Time. If your claim is processed after 2 p.m. Central Time, payment will be released the next business day.

Delta Dental programs allow you to select any licensed dentist. While we recommend you select a Delta Dental participating dentist because you may enjoy lower out-of-pocket costs and the convenience of having your claim forms handled free-of-charge by the dental office, you are free to choose any dentist. If your dentist is not a Delta Dental participating dentist, encourage him or her to become one.

We recommend parents bring their child in at or after the age of two, or at an age when they are most willing to communicate with an adult other than a parent. Early visits provide several advantages: they give us the opportunity to spot and prevent any potential oral health/dental problems; they can also give the child a positive, non-threatening dental office experience.

Bills sent directly by physicians and hospitals will be processed by Bollinger, Inc. However, after review, Bollinger, Inc may contact the Covered Person and ask him/her to complete a claim form or a questionnaire to get further information about the claim. Reinbursement for prescriptions must be accompanied by a claim form. Claim forms are available at www.BollingerInsurance.com/andrews File claims within 30 days of injury or first treatment of sickness.

The deadline for submitting a Claim Form under this Settlement passed on September 22, 2005, and no additional Claim Forms will be accepted.

Most of the time you will simply pay a reduced fee at the time of service, and there is no claim for this. However, if your plan includes limited insurance benefits (refer to your membership manual) you may need to fill out a form to file a claim.
