If you left the United States with your Form I-94 still in your possession, your departure was not recorded properly. If you do not correct this record, or if you cannot reasonably prove that you departed in a timely way when you next apply for admission to the United States, BCIS may conclude you remained in the United States beyond your authorized stay. Visit your International Student Advisor and report that the card is lost. You will need to file a Form I-102 Application for Replacement/Initial Nonimmigrant Arrival/Departure Document. Even though this card looks small, it is in fact a very important document. On an airplane, visitors are asked to fill out a Form I-94, Arrival/ Departure Record. At the port of entry (airport), students present this card to the immigration officer/ inspector. The officer takes the "Arrival Record" and gives you back the "Departure Record" or I-94, which is usually stapled inside your passport.

You will complete the I-94 upon your arrival in the US. An immigration officer will scan your I-20 (if you are an F-1 visa holder) and stamp your I-94. If you have an F-1 visa the officer will write "D/S" on your I-94. If you are here on a tourist or waiver visa the officer will stamp your expiration date in the I-94.
