Billing for physical therapy services is similar to what happens at your doctor's office. When you are seen for treatment the following occurs: The physical therapist bills your insurance company, Worker's Comp, or charges you based on CPT (Common Procedure Terminology) codes. Those codes are transferred to a billing form that is either mailed or electronically communicated to the payer.

When a domain is added to the system, your account is debited by 1 and the start date is set on the domain Just like when you buy a domain name, this debit is permanent unless you delete it within a few days. Every time 1 year or 200MB of usage is used (1 million DNS queries), 1 debit is added to your account for the zone When the number of debits exceeds the number of credits in your account, we will send a reminder notice to the billing contact listed on your account.

The announcement will also be forwarded to the email address of Authorized Payers. This will be sent each time a new statement with an outstanding balance is processed. We will continue to send paper bills to your address of record through Summer 2006.

Subscriptions are on a pay in advance system. Your first billing should arrive within a week of the start of service, and is due upon receipt. Your billing cycle will start the date you begin receiving service, and will proceed monthly from that date. Credits for missed delivery or vacation stops will extend the expiration date on your account by the number of days you received credit. Payments are due on the expiration date noted on your bill for the next month of service.

Every term (month* or year), your account is billed for the upcoming month of Pro service. This process continues until you cancel the service or your account experiences repeated payment failures (which causes automatic cancellation).

Once you have 'submitted' your credit card information using the Join Now menu, your card is automatically charged (the system is completely automated) unless it is rejected by the credit card company for some reason. If you think you have joined more than once in error, contact us (see bottom of this page). When you join, your credit card is simply billed for your membership at the rate for the period you choose.
