Several different techniques are used in the gastric bypass procedure, and surgeons have specific reasons for using certain gastric pouch sizes. While in the operating room, a test is typically performed to check for leaks before the surgery is completed. In addition, the following day, you will be asked to swallow a small amount of contrast dye while in the Radiology Suite to again determine if there are any leaks.

The surgeon is able to reduce the size of the stomach from two fists down to the size of a thumb, with a row of staples. The stomach is able to hold less, so the patient feels full very quickly. A part of the small intestine is re-routed, or bypassed, and the pouch is connected with a small opening (anastomosis) to the small intestine, which slows the progression of food through the GI tract.

For the first three weeks after surgery the only thing you can eat must be able to fit through a straw - tacos don't fit through a straw. The first four weeks are critical to healing. Some patients cannot tolerate certain foods they ate before the operation. Each individual is different. After four weeks most patients graduate to pureed foods.

On average, patients can usually expect to lose 30 lbs. within a month, 60 lbs. in six months and 100 lbs. in a year. Patients who exercise after surgery, as recommended, will experience better weight loss. People who are super-morbidly obese tend to lose weight at a faster rate. Yes, there are risks with any abdominal surgery. These risks include bleeding, the need for a blood transfusion, infection, the need for reoperation, injury to other abdominal organs, pain, complications outside of the abdomen, including the heart (MI or heart attack, arrhythmia), lungs (pneumonia), blood clots and death.

You will have 30 cc, or one ounce. You can have this several times a day, but only 30 cc at a time. Some can tolerate more than this without vomiting, but we recommend you be quite strict the first few weeks.

A roux-en-Y gastric bypass is a surgical procedure for achieving and maintainina healthy body weight. This procedure divides the stomach into a small gastric pouch, which will reduce the amount of food the stomach will hold. In addition, part of the small intestine is devided and attached to the small stomach pouch through a new small outlet. This rerouting causes the food to bypass the lower part of the stomach and a portion of the samll intestine where some digestive processes take place.

The Fobi Pouch is a Roux-en-Y gastric bypass with a band. (see diagram 2). The scientific name for The Fobi Pouch is the Vertically Transected, Silastic Ring Banded, Roux-en-Y Gastric Bypass with the Gastrostomy and the Gastrostomy Site Marker (TSRBVGB).

The Roux-en-Y helps you lose weight in three different ways: volume restriction, behavior modification and malabsorption. Volume restriction is achieved by reducing the size of the upper stomach. The Roux-en-Y procedure restricts the volume of food you can eat before feeling full. This means you will eat fewer calories, helping you lose weight. The changes in your small intestine create a behavioral modification component.

This operation has been performed for almost 20 years. The laparoscopic approach is relatively new. We have designed the laparoscopic procedure to be almost identical to the "open procedure".

Gastric restriction. The small stomach pouch created during surgery can hold only about two to four tablespoons (three ounces) at a time. In addition, the connection between this pouch and the jejunum is approximately 1.0 to 1.5 centimeters (1/3 to 1/2 inch) in diameter. Both of these factors drastically reduce the amount of food that can be eaten at one time. Nutrient malabsorption. Normally, the body absorbs calories and nutrients at all stages of digestion.

The term "Roux-en-Y" refers to the "Y" shape of the small bowel connection that is created during surgery. The small bowel is transected (divided) and then reattached at one end to the newly created pouch (the new stomach) where it will now receive food that has been chewed and swallowed. The other end is reconnected farther down on the small intestine, creating the "Y" shape. None of the bowel is removed during surgery.

This happens during the transition time. The transition time is typically between week four and six after surgery, although sometimes it is later. During this time you put foods into a blender or a food processor. This is when you can enjoy more. But you do this with the doctor's supervision. Some foods will still not be tolerated. During this time your food is more semi-solid. You still take in a limited volume of food.

During the first four to six weeks you cannot eat solid foods. You can eat solid foods once you have permission from Dr. Ali. After six weeks some foods will be more difficult to tolerate. Steak can be difficult for some to tolerate.

In some surgical procedures, the stomach is left in place with intact blood supply. In some cases it may shrink a bit and its lining (the mucosa) may atrophy, but for the most part it remains unchanged. The lower stomach still contributes to the function of the intestines even though it does not receive or process food - it makes intrinsic factor, necessary to absorb Vitamin B12 and contributes to hormone balance and motility of the intestines in ways that are not entirely known.

If you gulp water you can wash food out of the pouch and into the small bowel and lessen your feeling of fullness. Therefore you should avoid liquids for ? hour before and ? hour after meals after gastric bypass surgery.
