If you suspect your license tag was stolen or perhaps lost, you must file a police report and get a case number. Tags will not be replaced without that case number. Call the SJSO non-emergency number, (904) 824-8304 and ask for Communications to make a report. If you live in the City of St. Augustine, file your report with that police agency. Once you have the case number, you can apply for a new tag at the St. Johns County Tax Collectors Office.

If you have lost or damaged a license plate beyond recognition, then you need to replace both the front and back plates. If only one is damaged or missing, you must bring the other into a local county office. There, fill out and sign form MVR14, which is available at all local county offices. Take along your current vehicle registration certificate, because you will most likely receive a new version with the updated information.

PennDOT offers a person with disability plate that provides parking privileges and they offer a person with disability parking placard. (The placard hangs on the vehicle's rear view mirror and provides the same parking privileges as the person with disability plate.) The PBCC sends applications to PennDOT in batches around the 15th and the 30th of each month.
