Go to the State of Kansas website to request a duplicate W-2 if you are a current employee. For former employees, send an e-mail to payroll@ku.edu and request a duplicate form; be sure to include your name, current mailing address, and last 4 digits of your social security number.

First, contact your employer to find out if or when the W-2 was mailed. Allow a reasonable amount of time for the employer to re-mail or issue the W-2. Then, if you still did not receive your W-2, contact the IRS to prepare Form 4852 (PDF), Substitute for a Missing Form W-2.

Employees who need a replacement copy of their W-2 should come by the Payroll Office at 1600 Hampton Street. Please be prepared to show your University ID card and furnish your name, social security number, and the year for the replacement W-2.

The most common reason for failure to receive your W-2 is that Family Dollar does not have a current address for you. When you move, you must provide Family Dollar with your new address. Family Dollar mails W-2's to the last address the Corporate Office has on record. If you believe Family Dollar may have an incorrect address, please contact the W-2 Hotline (1-800-547-0359, Option #3, then Option #1) with the current address.

Upon availability, TAMUK employees will be able to access and print W-2 forms through HRConnect, which can be accessed thru Single Sign On (SSO). Scroll to the bottom half of the page and choose "Show W2 Form"--click the drop-down menu to check for multiple W2s. If you experience difficulty logging into SSO, follow the on-screen help or contact your Central Administrator (361-593-3706) to have your password reset.

If you cannot locate the employer and you have your last pay stub we can prepare a substitute W2 for you. A return may not be filed electronically with a substitute W2 until after 2/15/2007.

If you signed up to receive your W-2 form electronically, you will receive an email no later than January 31, 2006 that it is available in HRConnect. If you did not sign up to receive your W-2 electronically, the paper form will be mailed by January 31 and available in HRConnect by February 10.
