A copy of the debtor's discharge will be mailed to the debtor by the clerk's office after it is entered. If another copy is needed, please call the clerk's office (225 389-0211). **After filing, it is very important that the debtor retain his/her bankruptcy documents for future reference. Sometimes the information may be needed for some future purchases or other business transactions that will require proof of filing, list of creditors and discharge.

You may search for the cross reference or microfiche number by clicking on the Service Discharges link on the Recorder's Home Page and searching the database by name. Once you have this reference number, you may contact the Recorder's Office at (937)225-6494 and request a copy. Please keep in mind that Service Discharges are not required to be recorded in the County Recorder's Office.

The Department of Veterans Affairs does not maintain a record of veteran's military service, except as is necessary for providing benefits. For information about obtaining your military record, you can go to the website vetrecs.archives.gov. That page gives instructions in submitting a SF 180 Request Pertaining to Military Records or you can come to or call the Johnston County Veterans Service Office for help.

Call the Clerk's Office and a deputy clerk will instruct you on how to obtain copies of a discharge order. The instructions will vary depending on the location of the case file. The case file will either be at the Clerk's Office or the National Records Center.

Mail to the Court a written request with your name, case number and a stamped self-addressed envelope. Alternatively, you may go to the Court's website to complete a form (Coming Soon) requesting that the Discharge Order be faxed or e-mailed to you. If you do not have access to a fax machine or e-mail, please come to the Court at the Records Section with your name and case number.

Visit our Resource Center office - our staff will help you complete the necessary paper work. Once your discharge papers have been located, they will be sent to our office. Each patient is different. There are several factors to consider when evaluating your ability to return home: age, living arrangements, and insurance are the three main factors. Some patients go home with home care services, others go home with therapy scheduled in an outpatient setting, and some may go to a rehabilitation facility. Your doctor will help to determine what is best for you.

Normally you will visit with your doctor within 2 weeks after discharge. At that time, the surgical staples are removed, and the doctor will review your need to continue total joint precautions and therapy. The doctor will also address any specific modifications to your normal daily activities including work-related concerns, so bring a list of questions and/or concerns.

Most patients are able to go home directly after discharge. Some patients may transfer to a sub-acute facility and stay there for three to seven days. The Total Joint Coordinator will help you with this decision and make the necessary arrangements. You should check with your insurance company to see if you have sub-acute benefits.

We start the discharge plan at the onset of admission. We work very closely with your existing therapist, physician, psychiatrist and dietitian to coordinate your discharge. If you don’t have an existing team, we find one for you. Discharge plans will be formalized with appointments made before you leave.

Individual debtors are eligible to receive their chapter 7 discharge approximately 60 days from the date set for the 341 (a) meeting, unless a creditor objects in a timely manner or the court orders otherwise.

The quickest way to obtain a copy of your Veterans Discharge Papers is to visit this office. You must bring with you several forms of identification including at least one picture ID. The charge is $2.50.

If you do not have a copy of your discharge papers, service records, or medical records you can obtain them by filling out a Standard Form 180 - Request Pertaining to Military Records. You can download a copy from this site or obtain a copy from your local State Division of Veterans' Affairs office or the United States Department of Veterans' Affairs.
