We do not use an admissions formula or index system when we evaluate applicants, nor is there a floor below which we will not consider a candidate. When two non-prime numbers are involved in the ratio, it is possible for two or more libraries to tie on that measures. When ties happen, the total number of points assigned are skewed towards a higher number than would otherwise have occurred.

We do not have a quota to fill, so numbers vary from year to year (depending on how many students audition, etc.). We accept roughly 100 new students to Summer Session, out of the 600-1000 we see on tour.

If your score was significantly different from your practice tests (and you are confident that you were accurately timing yourself), and if you can identify something in particular that might have negatively affected your score the first time (you didn't prepare; you were sick; there was a marching band outside the exam site; et cetera), you should seriously consider retaking the test.

This is a very complicated question, the short answer to which is that your GPA / LSAT data will probably account for 80-90% of your chances of admission at any given school. These things vary by school, of course, and it is obviously artificial to speak in terms of percentages. The point, though, is that your GPA and LSAT score will form the core of your law school application. Most people would say that your grades are slightly more important that your LSAT score.
