Most immunization records are available on computer and can be printed out immediately. Microfilmed records can take up to two weeks to retrieve. There may be some instances where records are no longer available.

Health records are confidential. You can request a copy of your records in writing. The letter must state that you give the College of Staten Island permission to release your immunization and/or medical records. Then give the name, address, or fax number of the agency you want to receive your records. The letter must also be signed and include your student identification number. The letter can be faxed to us or you can go to the Health Center and fill out a Medical Record Request Form.

The Student Health Center keeps all health records, including vaccination records for a period of 10 years after you graduate or leave the College. To obtain a copy of your health records you must complete and submit a personally signed release of medical information form to the Student Health Center. If you made your request in writing for the records to be sent directly to you, the physician must provide copies to you within 15 days. The physician can charge a reasonable fee for the cost of making the copies. If the physician's office advises you that a fee will be charged for the records, the medical records do not need to be provided until the fee is paid. If the physician does not comply within the time frame you can file a written complaint with the Medical Board.

The test should take approximately 30 minutes. The CSI includes a brief questionnaire, to be filled out by the administrator and the patient, which covers general demographic data, medical information, and computer familiarity. Following the questionnaire is a series of computerized subtests that measure attention and working memory, learning capacity, and motor speed. At the clinician's discretion, an optional mental status examination may be added to the CSI.

Usually within two weeks after the records have been retrieved. Records with over 1000 pages may take slightly longer.

There are various retention periods for different types of health and safety records. We have published a list of the most common records which schools hold. Check with your family to see if they have a copy of your immunization records available. A copy of your records may also be on file with your previous place of education (high school, undergraduate university). Records may also be found at your doctor's office. Please be aware that these records may not be up to date. If you are sure that you have been immunized against the measles, mumps, and rubella, and/or the chicken pox, you have the option of getting your titers drawn.

A medical records release form is required to obtain a copy of your medical records. Records will be copied and mailed to you after the release form is received. There is a fee for this service. Please contact our office at (920) 730-8833 or toll free at (888) 383-3039 for more information.

Patients may request a copy of their medical records for their personal use or for delivery to a physician outside of Urology San Antonio by submitting the Medical Records Request form found on the Medical Records page of our website.

Talk with your dentist about getting a copy of your dental records. Dentists covered by the HIPAA privacy rule are required to provide patients with a copy of their records and state law may also apply.

The Broward County School District is a GED testing facility. All requests for verification of GED test scores and official GED transcripts are handled by the Florida Department of Education, GED Testing Office through their Web site (http://www.firn.edu/doe/workforce/ged_dipl.htm) or call them toll free at 877-352-4331.

You will need to send a written authorization, signed and dated by you, to our Health Information Management Department. An authorization is available for you to print from our web site.

It depends on the health status of the applicant and the health insurance company to which the applicant applies. Some health insurance companies may approve, within a few days, the application of a healthy young adult. However, for less healthy or older applicants, processing of an application can take several weeks or more. Each circumstance is different. You should consult your independent health insurance agent to get a realistic expectation.

The guidelines from the California Medical Association indicate that physicians must provide anything that they are maintaining in the medical record for you (as the patient), which includes records from other providers. So, for example, you should be able to receive a copy of a specialist's consultation report from your primary care physician, since he/she has incorporated it as a part of your medical chart.

Government employees can request access to their personnel records under the state Freedom of Information Act. There is no state law, however, which requires non-government employers to provide their employees access or copies of their personnel files. Non-government employees have a right to view or copy their personnel file only if company policy or an employment contract grants such a right.

The Medical Board may take any action against the physician which is appropriate for failing to provide the records within the legal time limit. This can range from making sure that the doctor actually does provide you the copy you requested, to a citation and fine or disciplinary action against the physician's medical license.
