On September 16, 2002 records of births and deaths which occurred within the corporate city limits of Hurst were transfered to the Tarrant County Clerks Office for Vital Statistics, Tarrant County Courthouse, Room 180, 100 West Weatherford St., Fort Worth, Texas, 76102. Even if you believe that you never had a birth certificate, you should follow the instructions to request a certified copy from the Tennessee Vital Records Office if you were born in Tennessee. If no record of your birth is located, the Vital Records Office will send you a letter certifying that no record is on file. A California Notary cannot certify a copy of a birth or death certificate, but may notarize an affidavit of a person requesting a certified copy of a birth or death certificate.
