You can now request a replacement red, white, and blue Medicare card online on Social Security's web site. Your card will be mailed within 30 days to the address SSA has on record. This service can be accessed during the following hours: Monday-Friday: 5 a.m. until 1 a.m. Your last (exact) payment amount or the month and year you last received a payment if you have received benefits in the last 12 months. This new service can be accessed via the Social Security Administration website.

If your Chicago Card Plus is lost, stolen or damaged, and you would like to obtain a replacement card, you should log on to your account at www.chicago-card.com and click on "Replace Card". Or you may contact CTA Customer Service at 1-888-YOUR-CTA (1-888-968-7282), Monday through Friday, 7 a.m. to 8 p.m. Once you notify the CTA that your CCP is lost, stolen, or damaged, your CCP will be turned off immediately so that it will no longer work.

Answer: Every application received by the NFIP Bureau and Statistical Agent will be responded to by mail only. You will receive information on the status of your application by receiving a rejection letter or the Flood Certification Number, as long as the printed mailing address is readable.

Visit the Campus Card Services office in the Clark Student Center, room 197 to replace your Campus Card. A replacement fee of $20 exists for all lost, stolen or damaged cards. Replacement fees can be paid with cash, check, credit card or Maroon Money.

Lost or stolen ATM or Debit Cards should be reported immediately by calling 1-800-666-4417 to minimize any possible losses. We will deactivate your ATM or Debit Card and issue you a new replacement card.
