A parent of the person named on the birth certificate (requesting parent's name must be on birth certificate)

For births in one of the five (5) boroughs of New York City (Manhattan, Kings, Queens, the Bronx, and Staten Island), please visit the New York City Department of Health and Mental Hygiene web site. Please note that the borough of Kings is sometimes referred to as Brooklyn and the borough of Staten Island is sometimes referred to as Richmond.

A person may obtain only his or her own birth certificate, except for parents who may obtain their own child's certificate. Access to birth records becomes available to the public 100 years after the event.

A birth certificate may be obtained from the local registrar of the city you were born in or from the New York State Department of Health. You may apply online at http://www.health.state.ny.us/nysdoh/vr/forms/birth.htm

You must either come in person to our office with picture identification, or submit a written request. The written request must include full name at birth, date of birth and both parents' names. You MUST SIGN your request and give an address where we are to mail the certified copy. The fee for the certified copy must accompany your written request. The request is filled the day we receive it and it goes out in the next business day's mail.

Only your current spouse, brother/sister, child/step-child or grandchild, parent/step-parent or grandparent, authorized agent, attorney or legal representative is authorized to obtain a certified copy of your birth certificate. The Application for a North Carolina Birth Certificate form completed.

Birth or death certificates or copies of the same are available from the Clerk and Recorder's Office at the Courthouse, 155 West Granite, Butte, Montana, Room 201. Certified copies of birth certificates are $5.00; death certificates are $3.00.

Alleghany County Register of Deeds keeps records only for births occurring in Alleghany County. Requests are honored by mail or in person. Payment is accepted in the form of a check or money order payable to the Registrar/City of Troy or cash.
