If wood is badly decayed, this will be quite obvious. The wood will be soft and perhaps even be breakable by hand. Decayed wood breaks with a carrot-like snap versus the splintering of sound wood. Use the pick test to be sure - see our page on Assessing decay.

Remove all decayed wood and additionally remove another two feet of sound wood all around the decayed section. Any sound wood that is left in place when decayed wood around it has been removed should be field treated with a penetrating preservative. A moisture meter will accurately measure the moisture content of your wood. You can roughly check the dryness of your wood without a moisture meter by knocking two sticks together. If you hear a sharp sound, the wood is probably dry. If you hear a dull thud, the wood is not fully seasoned. Cracking and checking on the edges of a stick of firewood also indicates that the wood is seasoned and dry.

The simple answer is, you build a frame, push the wood against it, and then screw the wood down to it, but you probably wanted better advice than that. For all but the top layer the wood should be bent with the grain. It bends much easier this way. The basic technique is to screw the bottom of your plywood in place, and have your helper(s) walk slowly up the transition bending it and holding it down with their weight against the frame as they go.
