You may print out your certificates of completion for the courses you have taken from "My Account" on the top menu bar.

Our policy is to issue a new card ONLY if you completed the course within the last 5 years. You may request a replacement card by emailing info@pamsp.com with your name, driver's license number, and the date and location the course was taken. IMPORTANT: We recommend that you laminate your completion card and keep it in a safe place. The Motorcycle Safety Program maintains records for class completion cards for only 5 years.

You may print duplicate certificates at any time for any course you have taken with ContinuingEdExpress.com. Simply login to your account and select the course.

We have an administrative fee of $15 for the first duplicate certificate and $5 for each thereafter. Please send us permission to charge your credit card the proper amount with your request for duplicate certificates.

ii. You may personally come to MII Customer Service Centre and get the Declaration of Loss Certificate form.

First, sign in to our site and navigate to "My Transcripts". There you will see a list of courses that you have passed. If you click on your chosen course it will display your certificate of completion for printing.

If you know your username and password you may print or email your certificate as many times as you like. Log into the MSB site as a return user. (Use the password reminder if you cannot remember your password.) On the homepage, click "Courses: Required to be Here" on the bottom left side of the page. You will see a list of courses. A certificate icon will appear next to each of the courses that you have completed.

A Certificate of Completion (C.C.) Is required for all substantial remodeling, renovations and rehabilitations without any change in use.

A Certificate of Completion (C.C.) can be issued after all applicable final inspections are approved, all required documents are filed with the Building Section of the Community Planning & Development Department and all applicable fees are paid.

Each court has their own rules regarding the manner in which they accept completion certificates. That’s why we tell you exactly how many copies of the certificate that you will receive, when you will receive them, and what you are supposed to do with each one.

CBNC can have a replacement certificate made for you. Please be aware that the CBNC office does not maintain copies of certificates and that the certificates are hand calligraphed from prior to 2006. Therefore, it will take 2-3 weeks from receipt of the request to have the replacement mailed. Cost for replacement is $30. Application must be made to ABC for a replacement license, and a fee of $5.50 paid for each one. (Section 24047) Yes, if the licensee has not paid an annual license fee. Otherwise, every license now in effect (other than temporary retail permits and special daily licenses) is renewable unless such license has been revoked after hearing on an accusation. (Section 24048)

If you have lost your share certificate you will need to inform our registrars as soon as possible. This can be done by telephone or in writing. Computershare will then be able to guide you through the process of obtaining a duplicate certificate. Depending on the value of the shares represented by the share certificate, a fee may be payable.

Download the Replacement Shingle/Membership Card Order Form from the web site (http://www.kkytbs.org/forms/ReplShingleCardOrderForm.pdf), complete it, and send it with your payment to National Headquarters. There is a $10 charge for replacement shingles, and (effective September 1, 2003) a $5 charge for replacement cards. As soon as we receive your order and payment, we'll print your replacement shingle or card and mail it to you.
